DOCUMENTS NOW ALL IN SNOMED FORMAT
EMIS web users - install all documents via the Protocols page
EMIS web users can install all the documents by installing the Diabetes Documents Protocol on the Protocols page. This is a basic Protocol. The more advanced Protocols will automatically load the correct version of documents. I am rebuilding these Protocols in SNOMED before releasing them. Diet Sheet not currently included.

Diet Sheet - First page adapted from Dr David Unwin's diet sheet
- Two page diet sheet
- First page adapted from Dr David Unwin's low carb approach as published in BMJ nutrition
- Second page is adapted from www.newforestpcn.co.uk/low-carb which is another website I have created
- Start using this leaflet for patients with Type 2 Diabetes, Pre-diabetes, Fatty Liver, Obesity and overweight.
- This approach has dramatically reduced our medication use and seen massive improvements in HbA1c and weight.
Type 2 Diabetes Care Plan
- Two page mailmerge document designed to be produced at the annual review appointment
- Two versions availble, one for those with a BMI over 25 (standard) and one for those with a BMI <25
- Document will need editing to contain your local logins for e.g. WW and phone number for retinal screening service etc.
- Annual Review Protocol automaticallty loads the correct version
- Areas in red may need editing, areas in blue need merge fields adding (word versions)

- Two page document designed to be given to patients suggesting or starting a statin
- Two versions available, one for those with Type 1 Diabetes and one for those with Type 2 Diabetes
- Diabetes Annual Review Protocol automaticallty loads the correct version in those indicated i.e. those aged over 40 with a Qrisk2 >10% who are not already on a statin

- One page document designed to be given to patients starting on Metformin
- Two versions availble, one for those with an eGFR of 30-45 and one for those with an eGFR >45
